Yes No VOLUNTEER FOSTER AGREEMENT— To serve as a foster for the American Wirehaired Pointing Griffon Association, you must agree to the terms and conditions of the AWPGA Rescue’s Foster Program listed below. Please confirm each of the following to signify your understanding and agreement: I certify that my own pets (if any) are currently licensed and up to date on all required vaccinations, including rabies. Confirm I will keep my pets (if any) separated from the fostered dog for at least 10 days to minimize the risk of my pets becoming ill or infected with any disease the fostered dog may be incubating. Confirm I will keep the fostered dog indoors unless accompanied outside. I will exercise the dog at least daily outdoors and will comply with all applicable leash laws. Confirm Should the fostered dog become ill or injured while in my care, I will call a veterinarian approved by my AWPGA Rescue contact person and will obtain preapproval from AWPGA Rescue for all veterinary care. I will facilitate communications between AWPGA Rescue and the veterinarian as requested by my AWPGA Rescue contact person. I also understand that AWPGA may contribute up to a total of $300.00 for shots, microchipping, spay/neuter, and heart worm medication. Confirm I understand that the fostered dog is the property of AWPGA Rescue. Confirm I will promptly return or deliver the fostered dog as instructed by AWPGA Rescue. Confirm I understand and agree that the American Wirehaired Pointing Griffon Association is not responsible, and shall not be held liable, for any property damage and/or personal injuries that may occur because of or in connection with the fostered dog or the fostered dog’s temperament or behavior. I will be fully responsible for the fostered dog while it is in my custody. I will indemnify and hold the American Wirehaired Pointing Griffon Association harmless against all liability, loss, cost, damage, and expense, including reasonable attorney’s fees, that may arise because of or in connection with the fostered dog while it is in my custody. Confirm I will indemnify and hold the American Wirehaired Pointing Griffon Association harmless should any animal(s) become ill from or injured by the fostered dog.
